TRICARE Online Releases Continuity of Care Document Published June 18, 2013 TRICARE Management Activity FALLS CHURCH, Va., -- TRICARE Online (TOL) recently released a new Blue Button download option called a Continuity of Care Document (CCD). The TOL CCD contains a summary of Blue Button personal health data including lab results, medications, allergies and problem lists. The CCD is an industry standard and is easy to share with other health care systems. The TRICARE beneficiary or patient can download Blue Button data into a CCD and share it with any health care system capable of accepting the file, including non-DoD health care systems. The CCD ensures the complete and accurate transfer of Blue Button personal health data. The CCD adds to the existing Blue Button portable document file (.PDF) and text download options available on TOL. It is very easy to use. TRICARE beneficiaries can download the CCD file to their computer and then upload it into their personal health record (PHR). Beneficiaries should note they may not be able to read the CCD on their computer because of the file type. If a beneficiary wishes to view the contents of the CCD file, he/she can easily view the same data in the .PDF download. Each of the TOL download options use the same data source and contain the same data as the CCD output. Beneficiaries should see step-by-step direction for sharing their Blue Button personal health data with the CCD below. How Do Beneficiaries Download the TOL CCD to their Computer? Step 1: Go to www.tricareonline.com and click "Log In" Step 2: Log in with their Premium Department of Defense Self-Service Logon, Department of Defense Common Access Card or Defense Finance and Accounting Services myPay account. If he/she does not have an account, click "Register". Step 3: Click "Blue Button" to access his/her personal health data Step 4: Click "Blue Button Download My Data" Step 5: Select the data types and the date range that he/she want to download, then click "Save as CCD" Step 6: In the open file dialog box, select "Save File", click "OK" and save in the location where he/she want to store the file on their computer. How Do Beneficiaries Upload the TOL CCD to RelayHealth? Step 1: Sign in to Relay Health Step 2: Click on "Health Records", click "Import/Export Health Data" Step 3: Click "Browse" to locate their CCD file and then click "Upload" Step 4: To view your CCD file, click on the file name link How Do Beneficiaries Upload the TOL CCD to Microsoft Health Vault? Step 1: Sign in to Microsoft Health Vault Step 2: Access "Health Information" from left navigation bar Step 3: Access CCD under the "Files" heading Step 4: Click the "Add Continuity of Care Document (CCD)" button Step 5: Click the "Browse" button to locate their CCD file then click "Save" Step 6: To view their CCD file, click on the file name under "Description" Please note, TOL tested and verified the transfer of Blue Button data via the CCD to RelayHealth and Microsoft HealthVault. However, due to possible variations in the CCD format and upload processes across industry; TOL can't guarantee the CCD will work with all health care systems.
