EFMP Program Information: The Exceptional Family Member Program helps identify and document special medical and/or educational needs of active duty family members and factors those needs into the assignment process. Family members who require special medical or educational services based on a diagnosed physical, intellectual, or emotional handicap must be enrolled in this program. Identified disabilities may range from mild to severe. Special needs may include special medical, mental health, developmental or educational requirement, wheelchair accessibility, adaptive equipment, or assistive technology devices and services. EFMP enrollment does not harm one's military career. The goal of the program is to consider a family member's special need during the assignment coordination process. Sponsors are still deployable according to the needs of the Service.
Mandatory enrollment for family members with any of the following medical/educational issues:
Forms needed for enrollment in EFMP program:
Family Member Relocation Clearance: Family member relocation clearance is a mandatory screening process that must be completed before Permanent Change of Station (PCS) orders are received for:
The checklist and information below should help ensure the process goes smoothly. Please keep in mind it does take time to collect the required documentation, schedule appointment with medical providers and the sponsor should contact the FMRC coordinator ASAP. This process should be initiated up to 6 months prior to your report no later than date. All forms are available through the FMRC office or on
www.afspecialneeds.org(requires CAC access). If you have a family member with a diagnosis of Asthma, ADHD/ADD, depression, anxiety disorder, bipolar, etc, or who is receiving care from a specialist (i.e. pulmonary, neurology, cardiology, or mental health provider) a DD form 2792 MUST be completed by the medical provider providing care. If your family member has never been seen in the Military Treatment Facility, then a DD form 2792 will need to be completed by their assigned primary care provider. If you need an appointment with a military provider please call
618-256-9355 to schedule.
Medical records maintained at the MTF will be reviewed for all family members needing clearance. Medical records from civilian providers must also be available for review for all family members needing clearance (sponsor, spouse, or majority age child will need to provide records). At the Relocation Screening appointment, all family members will meet with the FMRC coordinator, medical provider, and Special Needs Coordinator to review documentation and medical needs. If a family member requires special services from the gaining base then a Facility Determination Inquire (FDI) package will be sent to the gaining base for review of available medical/dental/educational services. The gaining base has 14 days to review the package and submit recommendations for approval/disapproval on family member travel. Once the FMRC coordinator receives travel recommendations back from gaining base the sponsor will be notified.
Documents Needed for Family Member OCONUS Travel AND for EFMP Family Members' CONUS Travel are the same:
Contact Family Member Relocation Coordinator once ALL forms are complete to schedule appointment for family clearance clinic. Relocation clinic occurs every Thursday morning 0830 - 1100 at the 375th Medical Group (2nd floor/TOPA conference room).
Special Instructions Regarding Remote, Unaccompanied, and Follow-On Assignments:

Once screening is completed, documentation is forwarded to the FMRC Coordinator inside the MTF where the sponsor is assigned. IMPORTANT: Sponsor WILL NOT receive orders until the above process is completed. Please start the medical clearance as soon as you receive your RIP (up to 6 months prior to reporting date) to ensure a smooth and timely transition.

The Referral Management Center (RMC) is a "one-stop" shop where patients go to process specialty referrals requested by their referring provider. The RMC can help set your Military Treatment Facility (MTF) specialty appointment. We can also help coordinate your off-site network provider if necessary. Additionally, the RMC can advise on health care benefits and review referrals.
Important Notes: If you have been "referred" by your Primary Care Manager, before leaving the clinic, please stop by the RMC. The RMC can assist with appointing you to an MTF specialty clinic for your initial visit. All follow-up appointments to the MTF sub-specialty clinic will be coordinated with that clinic and not the RMC. All MTF specialty referrals are considered expired if you do not set your appointment within our Access-to-Care standards.
If care cannot be provided on base, the RMC will defer you to our one of our TRICARE Health Net specialist medical provider. Come by the RMC, sit with one our referral managers and choose a specialist from a provider listing. All routine network referrals have an expiration date, so please set your initial appointment before the referral time period ends. The RMC cannot "renew", "modify", or "delete" an unused, unwanted, or expired referral. All expired referrals must be "renewed" by calling your PCM at
618-256-9355 (WELL) and following the tree to your assigned Family Health Clinic. For "continuity-of-care" referrals, please speak to your PCM team at
Duty Hours: For care considered Urgent, please call the
618-256-9355 (WELL) line for same day appointments to the MTF clinics. If an appointment cannot be made to the MTF, your PCM team will place a referral.
Non-Duty Hours: Please call the 618-256-9355 (WELL)
line and speak to the on-call provider from the patient's assigned Family Health Clinic. The provider will do an initial evaluation of your medical symptoms, and if necessary, may direct you to the nearest Urgent Care Center. If so, the provider will place a referral for your visit the next duty day. Please do not go to an Urgent Care Center without an authorization. Active duty members are required to seek medical care from their PCM and are required to have a referral authorization to visit an Urgent Care Center.
Emergency Care does not require a referral, but you are strongly advised to contact your PCM within 24 hours of ER/ED release at
