Together, you develop a plan of care to promptly control your illness, injury or situation, and navigate through the maze of medical care progressing toward you medical goal. Generally, patients with complex problems and considerable medical expenses receive case management support. Problems may be one or a combination of medical, social, financial, or mental health.

About us: The clinical pharmacists manage all anticoagulation therapy for patients empaneled to the 51st Medical Group. This includes ordering and managing their labs, prescribing and adjusting their anticoagulants, and educating patients on the importance of compliance, consistent diets, medication interactions, and the effects of alcohol on their anticoagulant medication. The goals of treatment will be based on the individual patient, but ultimately will be to keep the patient’s anticoagulation therapy at a safe and effective level, protecting them from complications.
