Patients are transferred as needed to the civilian hospital in downtown Minot.for emergency care. Ambulance Services is located on the first floor and provides 24 hour emergency coverage seven days a week to the entire base population. It is staffed with both active duty Emergency Medical Technicians and civilian paramedics.
About us: The Beneficiary Counselor provides you with assistance dealing with your TRICARE claims and collection agency activities. Services also include one on one counseling as related to your current and post service TRICARE benefits. If you are an active duty member considering elective surgery, you must stop by this office to obtain mandatory guidance as well as paperwork pertaining to your procedure prior to receiving the elective care.

About us: This is an Air Force mandated program for all active duty personnel with a special needs family member (e.g. spouse, child, or legal dependent). Emphasis is on general medical services, medically related services, and special education services. The program screens all Air Force families with an overseas assignment to ensure all necessary services are available at the gaining base. The Air Force Exceptional Family Member Program is designed to provide support to military family members with special needs. All branches of the military offer EFMP, and each Service includes a variety of personnel, medical and family support functions under the EFMP umbrella. About us: The Family Health Clinic provides acute, routine, and wellness care for the medical needs of TRICARE Prime beneficiaries, from birth to geriatrics. All enrolled beneficiaries are assigned a primary care,manager. PCM's may be a physician, a physician assistant or a nurse practitioner. We offer the following services: evaluation and treatment of most acute and chronic medical conditions; family planning (includes vasectomies and contraception), diagnostic services, minor surgical procedures such as ingrown toenail removal and skin biopsies, and a pseudofolliculitis barbae (shaving) clinic. PCMs refer enrolled patients to specialty care not be available in the 5 MDG. We provide walk-in services for pregnancy testing, suspected urinary tract infections, sore throats, and suture/staple removal. You do not need an appointment for a pregnancy test. You may go directly to the Family Health Clinic Front Desk and a nurse and/or technician will screen you, order required tests and/or take care of your concerns.

About us: The Mental Health Clinic mental health care for active duty and active Guard personnel. The clinic is staffed by psychiatrists, psychologist, social workers and licensed counselors. They provide mental health counseling, Alcohol & Drug Abuse Prevention & Treatment, the Behavioral Health Optimization Program, Drug Demand Reduction program, Suicide Prevention, Family Advocacy Program (including outreach), New Parent Support Program and numerous educational classes such as stress management, sleep hygiene, and anger management.

About us: The Optometry Clinic provides examinations, diagnosis and treatment for ocular and visual disorders. We are currently seeing active duty and dependents. Routine and same day care for acute eye conditions are available.
About us: Military Treatment records are the property of the U.S. government and must be maintained at the 5 MDG. Patients are not allowed to hand carry their medical records unless on PRP or PCSing to Korea. If you have a scheduled appointment on base, your record will be there for you already. If you have an appointment off base, please stop by and pick up a copy of the required documentation needed for your appointment.
About us: This program screens active duty family members for possible overseas assignment with their sponsor. A medical records review is conducted by each family member's primary care manager. Additionally, an Exceptional Family Member Program screening is performed.

About us: The Pediatrics Clinic serves your child's health care needs from birth to up to 18 years old. The pediatric clinic performs well baby visits, sports physicals, routine check-ups and acute appointments.
About us: The Physical Therapy Clinic treats patients based on a referral from your primary care manager or another qualified provider. The clinic performs exams, evaluations, and treatments for a full range of neuromusculoskeletal dysfunctions of our population.

About us: Our radiology department performs services via an authorized referral from your primary care manager. Routine procedures are provided on a walk-in basis. Other procedures such as MRI, ultrasound, mammography require an appointment.
About us: Referrals to an off base provider for care not available in the clinic are given to the patient by their primary care manager. Patients must then stop by the Referral Management Center to activate their referral, receive traveling instructions, and guidelines on reimbursable expenses.

About us: Services include transferring medical records, copying records, and the processing of all incoming and outgoing requests for medical information. All requests must be made in writing and must contain the appropriate legal authorization prior to release.
About us: By law, all military health care facilities must bill private insurance companies (third party insurers) for the care of military family members. It is mandatory that all non-active duty personnel complete a DD Form 2569, Record of Other Health Insurance, regardless of age, even if they do not have any other health insurance. Sponsors are not allowed to sign the forms for their spouse or any child that is 18 years or older due to HIPAA regulations. All monies collected from Other Health Insurance comes back to our facility to purchase items for the clinic in order to better serve you.

About us: The Women's Health Clinic provides a number of vital health care exams and counseling services for women. Although the 5 MDG does not provide obstetric care, this clinic does care for most other gynecological needs and issues affecting women, such as routine women's health exams and annual pap smears; evaluation for disease processes such as endometriosis; dysplasia clinic; and counseling on a variety of women's health concerns, such as birth control, infertility, hormone therapy, aging and menopause.
