About us: We provide an array of different services for the military. Flight Medicine provides clinical appointments for active and inactive fliers including sister service fliers. We offer Initial Flying Class Physicals I, II, III, Missile Operations, Personnel Reliability Program and Personnel Reliability Assurance Program. Individuals hired for civilian positions on base can receive pre-employment physicals. Base Operational Medicine Clinic performs record reviews for PHAs, security clearances, retraining overseas, and special duty clearances. Separation History and Physical Examination and Deployment Related Health Assessment are also performed by BOMC. For all physicals, please follow the instructions provided to you via email prior to your appointment. Please bring all appropriate paperwork since certain programs have specific requirements. The goal is to accomplish all requirements during your appointment and avoid additional appointments. To make this possible, please bring the following items to all exams: ID card; current glasses and sunglasses; most recent contact lens prescription, contact lens boxes or packages; and previous outside exam records, if possible.

About us: Our amazing team strives to provide the best evidence-based dental care possible for all patients. Our main priorities are the patient’s safety, satisfaction, and oral health. We make sure to keep our active duty personnel fit for the mission and our hope is to allow members to smile brighter after visiting. We provide comprehensive dental services to include exams, cleanings, and restorative work. We also provide limited care in the following specialties: prosthodontics, such as crowns and bridges; oral surgery, such as tooth extractions; periodontics, including treatment of the gingiva and supporting structures; and endodontics, such as root canals. We use “state of the art” technology including digital radiography and digital scanners and milling units to provide the best care to our patients. We can even deliver all ceramic tooth colored crowns the same day. If complex care outside the scope of practice of our dentists is needed, patients will be referred to the appropriate civilian specialist for treatment. The dental clinic serves active duty personnel on active duty status only. All beneficiaries without dental insurance may utilize services on a limited space available basis. Overseas clearances for dependents are approved and can be scheduled. It is highly encouraged that retirees and family members enroll in a dental insurance program. About us: Our operations are based on the Air Force Medical Service's Patient-Centered Medical Home model. Our goal is to provide you with the best quality, safest, and most customer friendly care possible. Neither location offers sick call services, however, same day appointments are generally available by calling the appointment line at 310-653-2873 (CURE). Per AFI 41-210, Sec 3.6.4., Unit Commanders and supervisors have the authority to grant up to 24 hours sick status at their discretion if a member’s illness or injury does not require military treatment facility intervention. Furthermore, the Family Health Clinic offers walk-in services for pregnancy tests, sore throat, injection, and wart re-treatment. Patients can be seen between the hours of 8 and 11 a.m. for these services.

About us: The Clinical Laboratory Flight offers limited services in chemistry, hematology, urinalysis, and serology. All other tests are shipped outside the facility to military and civilian reference laboratories. We accept requests from all off-base providers as long as the tests requested are on a lab requisition slip signed by the requesting provider and can be collected and performed by ourselves or one of our reference laboratories. The Clinical Laboratory Flight provides services to all eligible beneficiaries within the greater Los Angeles area. The laboratory does not provide results directly to patients. You can view your lab results on TRICARE Online or request results from your provider team via Secure Messaging, over the phone by calling 310-653-2873 (CURE) and choosing option 3 or ask the Family Health Front Desk.

About us: The vision of the Physical Therapy Clinic is to support the war-fighter by building healthy communities. Our mission is to return the injured military member to full duty as soon as possible. Our objectives are to support operations and contingencies, exceed customer expectations, promote injury prevention through education, provide evidence-based care and treatment, and enable the Physical Therapy staff to excel. We have one physical therapist and two physical therapy technicians. We are a direct access clinic; this enables us to accept patients for physical therapy treatment without a referral from a primary care manager or referring provider.

About us: We offer a variety of health services for the promotion and protection of health and control of disease. Our services include individual medical readiness health screenings, deployment and travel medicine consultation, communicable disease investigations, hearing testing, when occupational related, as well as food and public sanitation inspections. For Deployment Medicine support, our initial deployment processing are Wednesday, 1 to 3 p.m. Deployment final out processing is by appointment only, please call 310-653-6688. For all other Public Health services, please walk-in Monday through Friday, 7 a.m. to 4 p.m. PHAs are no longer conducted by Public Health, they are completed by BOMC. BOMC can be contacted at 310-653-6850. A web-based PHAQ is an annual requirement for all Air Force personnel, and must be completed within 10 days of ASIMS notification. Airmen who do not complete the AF PHAQ within the specified time period shall receive a follow-up notification from their Unit CC, First Sergeant, or UHM. About us: X-ray procedures never require an appointment; they are done on a walk-in basis Monday through Friday, 7 a.m. to 4:30 p.m.; no lunch closure. Children are not allowed into the exam room, so childcare arrangements should be made ahead of time. There are no preparations prior to routine X-ray examinations. Comfortable clothing is recommended but not necessary since the possibility of having to change into a gown for an exam does exist. Disposable gowns and shorts are provided when necessary. Radiology does accept prescriptions for X-ray exams from approved United Healthcare providers. Providers will submit any specialty imaging request to the Referral Management Center. Specialty exams include mammogram, CT, ultrasound, nuclear studies, and bone densitometry. Once a referral has been submitted, status can be checked after two business days by calling 844-866-9378 or going online to TRICARE West. Patients can access their referral and also locate imaging facilities in their area for scheduling. Diagnostic Imaging Services is closed the first Thursday of every month from 7 a.m. to 1 p.m. for squadron training. Copies of imaging without the report are available immediately after the exam. Final reports are available the following day. Please refer to the ordering provider for results, the radiology department cannot release or discuss results with patients.

The Outpatient Records Department is responsible for managing and safeguarding medical records for all patients assigned to the 61 MDS. The Release of Information section provides copies of medical records to patients and other agencies upon request. Routine record requests will be completed within 20 working days. Upon receipt of a member’s PCS orders, the complete medical record will be mailed to the member's gaining military treatment facility. If an active duty member retires or separates, the medical and dental record is sent to the Service Treatment Record Cell located in San Antonio, Texas. The Referral Management Center processes patient specialty referrals requested by the military treatment facility primary care manager. The RMC will route your referral to the requested specialty within the MTF or to Health Net Federal Services for treatment by an off-base specialist.
In some cases, we can also refer our patients to nearby MTFs for services that we cannot accommodate.
