About us: We provide chiropractic care to active duty personnel. The Chiropractic Clinic gives special attention to the physiological and biomechanical aspects, including structural, spinal, and musculoskeletal. Our service specifically includes the adjustment and manipulation of the articulations and adjacent tissues of the human body, particularly of the spinal column. All appointments are scheduled through the Chiropractic Clinic phone number located above. Care is provided by a government contract civilian chiropractor.

About us: We perform screenings to detect the presence of drugs to protect the Air Force mission and public safety. In addition, DDR offers prevention material, briefings and presentations to spouse organizations, community groups and leaders to educate on the dangers of drugs.

About us: The Exceptional Family Member Program is designed to provide support to military family members with special needs. All branches of the military offer EFMP, and each Service includes a variety of personnel, medical, and family support functions under the EFMP umbrella. This program ensures services are available to meet the needs of your family at your assigned location.
About us: We provide family maltreatment prevention and treatment services to active duty and their family members. FAP programs center around prevention and outreach, treatment, victim advocacy, education, and research. Within FAP is the New Parent Support Program, which is a voluntary program for expectant parents and families with children under the ages of three. The program includes home visitation services as needed. Family Advocacy Strength-Based Treatment is another voluntary program for couples and families who desire to change destructive patterns of communication. Outreach services are offered through life-skills classes in parenting, relationship enhancement, emotion regulation, and resiliency boosting. Treatment services are offered for individuals, couples, and families who have experienced maltreatment.

About us: We treat all acute illnesses, perform personal health assessments, and offer clinical procedures such as toenail removal, IUD insertion and removal, and shave biopsy. Walk-in services include: sore throat; pregnancy test; injections of B-12 and Depo-Provera; ear irrigation; wart treatment; dysuria and UTI for pain, burning with urination, urinary urgency and/or frequent urination. B-12, Depo-Provera and wart treatment require initial provider visit. We also make referrals to off base providers as needed.

About us: We offer the full range of acute, routine and preventive medicine services. We also manage the Occupational Medicine Program and perform occupational health physicals on active duty personnel, annual flight physicals, pre-employment physicals for civilian members working for the 377th Air Base Wing, perform Initial Flying Class physicals, and firefighter physicals. We also take care of the permanent party and students/trainees in the 58th Special Operations Wing. We conduct industrial hygiene visits and perform food inspections on base. We also take care of all active duty PHA’s and MHA’s. Our Medical Standards Management Element handles all overseas, retraining, and Professional Military Education, Separation Health Physical Health medical clearance requests and record reviews, 469 and 422 profile reviews.

We may be unable to provide travel vaccines when supplies are limited as active duty personnel will have priority during those times. If you are interested in starting allergy immunotherapy, please schedule a consult with your primary care manager.
About us: We offer routine laboratory draws. We draw labs for off-base providers for TRICARE beneficiaries. Glucose tolerance tests are offered by appointment only. Please note for laboratory results individuals will need to contact their primary care manager.

These walk-in services do not guarantee that the patient will be seen by a provider.

About us: Disability evaluation begins only when examination, treatment, hospitalization, or substandard performance result in referral by your attending physician for an MEB. You will be referred to the Patient Administration Office where you will meet your Physical Evaluation Board Liaison Officer, who will enter you into the DES.

About us: Patients can be seen without a referral from their primary care physician for the following diagnoses: musculoskeletal evaluation; orthopedic rehabilitation for pre-operative and post-operative; and athletic injuries. Some of the services we provide include modalities, dry needling, rehabilitation training, specialized muscle strengthening, postural evaluation, exercise prescription, manual therapy, and acute and chronic pain management. When you get a referral from your primary care manager, please come to the Referral Management Center before departing the hospital. If the specialty is available at the 377 MDG, the clinic will contact you by phone to schedule an appointment. If the specialty is available at the VA, we will send your referral to the VA and they will contact you by phone or mail to schedule an appointment. If the specialty is with a civilian specialist in the network, your referral will be processed by Health Net Federal Services. About us: We schedule 20 minute wellness exams to update pet vaccinations, heart worm tests, microchips, and address minor concerns. We also schedule 30 minute sick call and senior wellness exams. All appointments are on a first come first serve basis, and must call to schedule the appointment in advance we do not see walk-ins. All prescription refills we require 24 hour notice.
About us: We are a self-referral clinic that provides routine and specialty women’s health care to active duty, dependents, and TRICARE Prime enrollees. Available gynecological services include general annual exams with pelvic exams and pap smears, pelvic pain, menstrual concerns, contraceptive management, such as birth control (Birth Control). We also provide screenings for pregnancy and vaginal infections.
