About us: School and sports physicals for children are performed by their primary care manager. The rush period is July and August because of the new school year, fall sports teams holding tryouts, and people moving into the area who need school physicals. Please try to schedule your child's physical early. Availability is limited the longer you wait. If your child has already had a physical in the last year, simply drop off the required forms for the primary care manager to complete. An annual physical is good for 365 days. We also conduct the following: 9 month screening, 18 month screening, 30 month screening, autism screening, autism screening follow up, District of Columbia school physical form, Maryland school physical form, and Virginia school physical form.

About us: We provide preventative medicine through immunizations to prevent the spread of infectious disease. Tuberculosis tests, or TB, are administered every day except Thursdays and the day before a holiday weekend.
About us: We perform 44 tests locally, and regularly send samples to outside labs for testing. Patients can receive results within 10 business days from the date of collection through their health care provider, at the front desk of the main clinic, or at Patient Administration located in Building 3, Room 204.

Records must be returned to the 11th Medical Squadron Outpatient Records for appropriate filing and maintenance. If you would like to request a copy of your records, please stop by the 11 Medical Squadron Patient Administration desk in Building 3, Room 204, to fill out a release of information form, DD Form 2870, or call for any records-related issues.
