To make an appointment, log in to the TOL Secure Web Portal or call 801-728-2600, Monday through Friday, 7 a.m. to 4 p.m. We are closed the second Wednesday of every month.
For the Nurse Advice Line, call 1-800-TRICARE (2273), option 1. If during duty hours, contact the primary care manager team at 801-728-2600. If after hours, call 801-728-2600 or 1-800-874-2273 and follow the prompts to the Nurse Advice Line. Patients must receive a verbal authorization from the Nurse Advice Line prior to an urgent care clinic visit.

For any care that your primary care manager does not provide, such as urgent, preventative, mental health, and specialty care, you will need a referral. Find out more about how to obtain the necessary referral and authorization.
