About us: ADAPT services are designed specifically for active duty personnel to promote health and wellness through the prevention and treatment of substance abuse. Services include individual and group treatment for military members who have been evaluated and diagnosed with substance abuse or dependence. We assist participants to attain and maintain sobriety, utilize support networks, and change self-destructive behavioral patterns. Services for non-active duty TRICARE beneficiaries are available off base, but require prior authorization.

About us: The Hearing Conservation Center provides annual audio testing to more 5,000 Hill Air Force Base workers who are occupationally exposed to hazardous noise. The Audiology Clinic provides diagnostic hearing testing and hearing aid services for active duty personnel only. Retirees and dependents of all ages can be seen for diagnostic audiology services on a space available basis.

About us: We are co-located with Flight Medicine and provides occupational medicine support to all military members enrolled at the 75 MDG. We oversee enlistment, commissioning, initial flight physicals, Annual Preventive Health Assessments, duty limiting condition profiling, PCS medical clearances, and Separation/Retirement Health Assessments support to military members.

About us: Case management is available for our patients with complex medical needs, such as catastrophic injury or illness, chronic or terminal illness, or multiple medical problems. The goal is to help coordinate your medical needs and assist you with achieving your self-care goals. Referral for case management can be obtained through your primary care manager or you can self-refer.
About us: We provide dental services to active duty personnel only. Services include dental examinations, preventive and restorative dentistry, prosthodontics, endodontics, periodontics and limited oral surgery capabilities. Active duty personnel must receive a periodic dental examination to assess each member's readiness status. Appointments for periodic dental examinations are scheduled by the Dental Clinic and coordinated with the Unit Health Monitor who informs each member of their pending appointment. Group First Sergeants and/or Commanders will be notified of all broken appointments. Identified treatment needs will be scheduled after the periodic dental examination. Active duty family members are eligible to enroll in the TRICARE Dental Program.
About us: Disease management clinic is available for our patients with specific high risk medical conditions. The goal is to improve your health outcomes and establish self-management. Pre-diabetes, diabetes, asthma, hypertension, and high risk hyperlipidemia are the conditions currently managed. Contact your primary care manager for a referral if you have these conditions or feel free to visit the Disease Management Clinic for education and counseling.
About us: We medically clear all active duty family members to travel during a Permanent Change of Station. We ensure the availability of medical care at all gaining bases, as well as dental and educational services at overseas gaining bases. We also assist families with case management care coordination for humanitarian reassignment requests and transitioning special needs children into adulthood with the creation of medical sufficiency statements for insurance purposes.

About us: The Family Advocacy Clinic is the primary office for preventing and treating family maltreatment involving active duty members and their families. Services include new parents support program, pregnancy classes, anger management, parenting class, marital therapy, outreach programs and therapy for abuse cases, victim and witness assistance program, crisis counseling, and referral resources.
About us: We provide primary medical care to beneficiaries from birth to geriatric age for active duty and their family members and retirees and their family members. Available services include diagnosis and treatment of acute illness, chronic disease management, well-child care and adult physical exams, developmental and behavioral evaluation, and preventive health care. Family Health nurses are also available during duty hours to provide medical advice via TRICARE Secure Messaging or telephone regarding at home care for minor conditions and illnesses. If you have questions, send a TRICARE secure message or, if you prefer, call our appointment line at 801-728-2600, choose the book an appointment option, and request a consult with your nurse. Your message or call will be addressed as soon as possible. Requests for medication renewals require 72 hours to process.
About us: We provide services and care for military members on flying status and special operations personnel and their family members. Services provided include routine care of acute illness and injury, minor surgery, and preventive health care and counseling. Flight Medicine also completes all Preventive Health Assessments for personnel on flying status, formerly known as the Annual Flight Physical.

About us: We provide health and wellness services designed to promote healthier behavior, enhance resiliency and human performance, and reduce illness and injury. Services include squadron focused Health and Readiness Optimization Reports which assist with strategic plans and squadron specific presentation to address areas found to be in the higher risk categories. The staff also assist individuals with body fat assessments, injury prevention running assessments, fatigue mitigation and sleep hygiene, and nutrition including, but not limited to, information and counseling on obesity and cholesterol reduction, hypertension, and general weight loss.
About us: We provide immunizations on a walk-in bases for all eligible beneficiaries during normal duty hours. Tuberculosis skin tests will not be administered on Thursday or when the 48 to 72 hour reading window falls on a holiday, down day or training day. The last vaccination is given at 4:15 p.m. to allow for a 15-minute observation period after the vaccine is given. Small pox clinic is conducted every Wednesday from 2:30 to 4:30 p.m. No other vaccines will be available at this time. Immunization record requests should be marked Attn: Immunizations and faxed.
About us: We provide services for eligible beneficiaries. The laboratory collects and tests patient specimens upon written request from your primary care manager. Test results are returned directly to the provider ordering the test(s). Your primary care manager will contact you with the results. Additionally, you can retrieve your results via TRICARE Online or at the Medical Records Office.
About us: The MEB Physical Evaluation Board Liaison Officers assist active duty Air Force, Guard and Reserve members through the Physical Evaluation Board process. Other services provided by the MEB Office include: Line of Duty Determination; Review-In-Lieu-Of MEB; and Temporary Disability Retirement List (TDRL). Please call to schedule an appointment.
About us: We maintain all hard copy and electronic health records. In addition, medical records also completes all new patient registrations for the clinic, including newborns and patients being seen in the clinic for the first time. Release of Information maintains all medical record copy requests. These include requests from off-base providers, beneficiaries, and retiring or separating members. Per AFI 41-210 medical record copy requests can take up to 30 calendar days to process.
About us: We enhance the health and readiness of the community by providing mental health assessment, education and outreach, consultation, and treatment services, such as individual and group psychotherapy, medication management, to active duty personnel. Non-active duty adult TRICARE beneficiaries can obtain care in the civilian community with an approved TRICARE provider and do not need a referral or prior authorization.

About us: We provide health services for all eligible personnel, including civilian personnel. These services aid in the prevention of occupational disease and illness, occupational surveillance of the worker and worksite, care of employees with work related injuries and illnesses, and support for the Air Force mission to fly and fight. Occupational Medicine also offers three board certified occupational medical physicians, two physician assistants, a full time clinical psychologist, reminders to you and your supervisor of upcoming appointments, shop visits with a physician, nurse, psychologist, and audiologist, and an employee evaluation system to ensure an expedited and safe return to work. Services include: occupational injury and illness evaluation, treatment, and follow-up; psychological assistance for work related stress problems; occupational surveillance exams; respirator fit testing and physical evaluation for respirator wear; occupational vision and hearing evaluations; occupational shop visits; fitness for duty evaluations for physical and mental and emotional problems; participation in the Light Duty, Physically Disqualified Program, and OWCP programs.
About us: We provide routine and urgent eye care to TRICARE Prime patients to include active duty personnel and dependents, with limited slots, and NATO service members. Routine or annual eye exams can be scheduled by calling the 75 MDG central appointment line. Glasses and other spectacle devices can be ordered for active duty and Reservists and Guard members on orders for more than 30 consecutive days, and retired military members. To receive a contact lens prescription, patients must currently wear contacts and bring them and the boxes or written prescription to their appointment. Refractive surgery pre-operative evaluation and post- operative care are available for any military member who qualifies for the USAF Corneal Refractive Surgery program as determined at the member's routine eye exam. Active duty family members, retirees, and their family members are eligible for the Federal Employees Dental and Vision Insurance Program. About us: We serve as the Air Force Medical Home for beneficiaries from birth through adolescence. Available services include diagnosis and treatment of acute illness, chronic disease management, well-child care and sports physical exams, developmental and behavioral evaluation, and preventive health care. Well-child evaluations are done at age 3 to 4 days, 2 weeks, 2, 4, 6, 9, 12, 15, 18, 24, 30, and 36 months, and yearly thereafter. Pediatric nurses are also available during duty hours to provide medical advice via TRICARE Secure Messaging or telephone regarding at home care for minor conditions and illnesses. About us: We provide evaluation and treatment of neuromuscular and musculoskeletal conditions to improve movement and function and maximize patient potential for an active and independent lifestyle. Priority is given to active duty personnel, with space available services provided to TRICARE Prime enrollees. All other categories will be eligible for services in accordance with 75 MDG policy. Referral by primary care manager or specialist is required for evaluation and treatment. Physical therapy appointments for active duty personnel are scheduled by the Referral Management Center at 801-586-9571 or 801-777-5414.

About us: We provide the following services for all beneficiaries: food safety and public facility sanitation inspections; communicable disease surveillance, education, and prevention; and travel medicine services, specific country vaccination requirements, current disease outbreaks, personal protective measures, etc. Specific services for active duty personnel include pre- and post-deployment medical screening and clearance, to include civilian deployers; pre-deployment, pre-separation, and flyer audiograms; and fetal protection education and work restrictions.
About us: We provide routine digital diagnostic X-rays for all eligible beneficiaries. Exams will be performed on a walk-in basis. Examples of services not performed are MRIs, CT scans, ultrasounds, and mammography.

About us: We process referrals for 15,000 beneficiaries. Referrals are generated by the 75 MDG providers to network specialists. Referrals are then reviewed by the Utilization Nurse and forwarded to the RMC to send to Health Net Federal Services for authorization. Once the patient completes their network appointment, the specialist sends the results to the RMC. If the RMC staff does not receive notes from the specialist, they will contact the civilian provider to request the results. To obtain or renew a referral you must contact your primary care management team. Health Benefits provides all TRICARE beneficiaries assistance with claims, enrollment issues, TRICARE eligibility, and general information regarding all aspects of the TRICARE Program. The Debt Collection Assistance Officer in the Health Benefits Office is available to provide assistance with TRICARE-related debt collection issues. About us: Women’s Health Services are provided through the Family Health and Flight Medicine Clinics. Care is provided by Women’s Health Nurse Practitioners. Services available include: well woman exams, or Pap smears); birth control; clinical breast exams; pregnancy testing; treatment of vaginal infections; care of abnormal Pap smears and treatment of menopausal symptoms. Obstetrical care and care requiring surgical intervention are referred to the civilian medical network.
