The Family Health Clinic supports active duty, active duty family members, and all other beneficiaries as determined by the Medical Group Commander.
Hours: Monday through Friday, 8 - 11 a.m. and 1 - 3 p.m.
Walk-in Services: Urinary tract infection, Sore Throat Clinic, Testosterone and B12 injections, follow up wart removal (no initial treatment), suture removal, and non-emergent medical related PT test failure (may walk-in to Physical Therapy first).
Quarters: Active Duty members given quarters by an off base provider must bring paperwork to the Family Health Clinic front desk for approval/validation. The front desk will provide a working copy for unit leadership. They may also attach to a MiCare message. UDM/UHM and First Sergeants can access quarters authorizations in ASIMS.
Convalescent Leave: Upon notification of surgery, please send a completed AF Form 988 via MiCare. Please include specialty provider's medical notes, restrictions and dates. The AF Form 988 will be processed and returned via MiCare with three (3) business days. The form and notes may also be dropped off at the Family Health front desk. The member or First Sergeant may pick up the form, after three (3) business days and route accordingly.

Flight and Operational Medicine Clinic provides comprehensive healthcare for Flight Crew and their family members.
The Air Force Medical Service is restructuring the Flight Medicine Clinic by separating primary care and occupational medicine services into two distinct clinics. In addition to providing primary care services, Flight Medicine is also the location of the Base Operational Medicine Clinic (BOMC), where the primary goal is to perform flying physicals, occupational exams, and deployment related services. BOMC is also the location where separation physicals, retraining, and overseas clearances occur.

Return to Fly Clinic: 10 minute appointments to return aircrew members to flying status after being grounded. Return to Fly Clinic Hours are Mon-Thurs 0730-0800 and 1300-1330 and Friday 0830-0900.
Flight Surgeon On-Call: A Flight Surgeon can be reached 24/7 for emergent/ urgent flight crew issues at 325-829-9131 or by calling the Command Post at 325-696-1921.
OFF BASE CARE: Flight Crew members that are seen at an off-base urgent care, the ER, or a specialist will need to be evaluated by a Flight Surgeon prior to resuming flying duties. If you are seeing a specialist, click here to download a copy of the form that the specialist needs to complete at each visit.
FAMILY MEMBERS: Flight Crew dependents (spouses and teenagers 15 and over) can be empaneled to Flight Medicine by calling Tricare at 1-800-444-5445
PERIODIC HEALTH ASSESMENT (PHA): The annual flight physical for aircrew (PHA) must be accomplished every 12 months, not 15 months. Complete your online medical assessment (WebHA) prior to booking your PHA appointment. Bring your glasses and/or contacts with you to your appointment to avoid being rescheduled.
INDIVIDUAL MEDICAL READINESS (IMR): Flight Crew can check their IMR to see the status of any vaccinations, lab work, or appointments that are due. View MyIMR.

The Pediatric Clinic provides care for patients ranging from newborn to children fifteen years old for a variety of reasons.

The Pharmacy will also be closed 7:30 a.m. to 12:30 p.m. the second Friday of every month for training and open for normal operations from 12:45 p.m. until close of business.
